Address/Name Change
Address Change
- To change your address (both permanent and temporary):
- You may call 574.257.3325 or e-mail with your new information.
- You may also stop by the registrar’s office to fill out a form.
Name Change
To change your name you need to bring (or send by mail) legal documentation of your new name to the Registrar’s Office . Acceptable documentation includes:
- New SS card
- New drivers' license
- Marriage license (legal license not ceremonial)
- Divorce decree
